Early learning coaches sent a newsletter to schools as a template for licensor visits that included the following information.
Some postings that are required by licensing:
- No Smoking and No Vaping Signs
- Hand washing poster by every sink
- Diaper-changing procedures in diaper-changing areas
It also offers a list of items in children and staff files checked by licensors. Please review this attachment to ensure your program has appropriate items for all files.
CHILD FILES:
- Registration Form
- Childcare Agreement
- Signed Parent Handbook and Policies Agreement Form
- Up-to-date Immunizations Record
- General Permissions and Authorizations Form (Consent for photo/video activity, water activity, bathing if needed/overnight care, transportation)
- Allergy List (if applicable)
- Medication Authorization (if applicable)
- Individual Care Plan- ICP (if applicable)
STAFF FILES:
- Current ID – Portable Background Check
- Current Resume
- Signed Staff Handbook Policies Agreement
- Documentation of Training Provided to Staff
- Documentation of Certificates
- Copy of Education Credits/Transcript
- Up-to-date TB Test
- Proof of COVID Vaccine (no longer required as of Nov 1)
