Washington State law requires all public and private K12 schools and preschool programs to file an Immunization Status Report to the Department of Health.
Schools have two reporting options:
1. School Module – DOH preferred reporting method
An active user of the School Module (WA ISS database) does not have to file a report. The report will be drawn from School Module data.
2. Direct Report
On November 1, an email about the annual immunization report was sent to school administrators. The email included an instruction worksheet, training documents, and a unique link for each school to submit using the RedCap online reporting tool. The school administrator can forward the email to the person designated to complete the report. If your school did not receive the reporting email, you should contact OICPSchools@doh.wa.gov.
How to submit your Direct Report:
- Use the K-12 School Annual Report Worksheet (PDF) OR the Child Care-Preschool Annual Report Worksheet (PDF) to gather the information for the report.
- Use the unique link emailed to your school or program administrator on November 1st to open the survey and complete the report.
All immunization information must be submitted by December 1st.
